New Patients
For appointments, please call 1-406-728-6101. You can also submit a request for an appointment online.
Please bring the following to your appointment:
- Photo ID
- Insurance card(s)
- Medication/Allergy list
Medical Information Release
If you need a copy of your medical record or x-ray, please complete the Authorization for Release of Medical Information and return the form to our office. Please allow seven business days for all requests.
No Show/Cancellation Policy
Please arrive for your appointments on time. If you are more than 10 minutes late, you may need to wait until the provider is available to see you or you may be asked to reschedule. Northern Rockies Orthopaedics requires a 24-hour notice in the event of a cancellation. To cancel an appointment, you must call Northern Rockies Orthopaedics during business hours (7am-5pm Monday through Friday). Our after-hours emergency line/answering service is not appropriate for cancellation notification, and the message will not be relayed to the Northern Rockies Orthopaedics staff.
Effective 1/1/2024, there is a $50 charge for all no-show appointments and/or less than 24-hour notice of a cancellation; in-office procedures will be charged a $200 fee. This charge is not covered by your insurance and is due prior to the rescheduling of any appointment that was missed. At the Providers’ discretion, missed or cancelled appointments may result in the dismissal from Northern Rockies Orthopaedics.
