Please bring the following to your appointment:

• Photo ID
• Insurance card(s)
• Medication/Allergy list

Medical Information Release

If you need a copy of your medical record or x-ray, please complete the Authorization for Release of Medical Information and return the form to our office. Please allow seven business days for all requests.

Health Insurance Portability and Accountability Act

Be assured that our office complies and takes every precaution to protect the privacy of your individually identifiable health information. Before our office releases information to any individual we will verify information you asked to provide to our office.
